- Health Information Custodian: Martindale Animal Hosptial, including our veterinarians and staff, acts as a custodian of personal health information as it relates to veterinary care.

We collect only the necessary personal information required to provide high-quality veterinary care. This may include:
- Pet’s medical history
- Vaccination records
- Diagnostic results
- Owner’s contact and billing information
This information is collected directly from the pet owner or authorized representatives, except where the law permits otherwise.

The PHI collected will only be used for the following purposes:
- To diagnose and treat the animal.

Clients have the right to request access to their pet’s health records or correct any inaccuracies in their personal information. All requests must be made in writing, and we will respond within a reasonable timeframe, as specified under PHIPA guidelines.
Retention and Disposal of Personal Health Information
PHI is retained as long as necessary to fulfill the purposes outlined in this policy or as required by law. Once the information is no longer required, it will be securely disposed of following PHIPA’s regulations, ensuring it cannot be accessed by unauthorized individuals.
Safeguarding Personal Health Information
We take the following steps to protect personal health information:
- Implementing physical security measures, such as locked filing cabinets and restricted access to offices.

Complaints
If a client has concerns about our handling of their personal health information, they may contact us directly to resolve the issue. If the concern cannot be resolved, clients may contact the Office of the Information and Privacy Commissioner of Ontario (IPC) to file a complaint.
