  • Personal Health Information (PHI): In a veterinary context, PHI includes details about the health history, treatments, and care of an animal, as well as the personal details of the pet owner, such as name, contact information, and billing data.
  • Health Information Custodian: Martindale Animal Hosptial, including our veterinarians and staff, acts as a custodian of personal health information as it relates to veterinary care.

Collection of Personal Health Information

We collect only the necessary personal information required to provide high-quality veterinary care. This may include:

  • Pet’s medical history
  • Vaccination records
  • Diagnostic results
  • Owner’s contact and billing information

This information is collected directly from the pet owner or authorized representatives, except where the law permits otherwise.

Use of Personal Health Information

The PHI collected will only be used for the following purposes:

  • To diagnose and treat the animal.
  • To communicate with the pet owner about their pet’s care.
  • To comply with regulatory and legal requirements.
  • For billing and insurance claims where applicable.

We will not use PHI for any other purposes unless authorized by the client.

Disclosure of Personal Health Information

We will only disclose PHI to third parties in the following situations:

  • With the pet owner’s consent.
  • To another veterinary hospital or specialist for continued care of the animal.
  • When required by law, such as in cases of suspected animal abuse.
  • To insurance companies for claims purposes.

We ensure that any third-party service providers we work with (e.g., labs or insurers) comply with privacy obligations as required by PHIPA.

Access to Personal Health Information

Clients have the right to request access to their pet’s health records or correct any inaccuracies in their personal information. All requests must be made in writing, and we will respond within a reasonable timeframe, as specified under PHIPA guidelines.

Retention and Disposal of Personal Health Information

PHI is retained as long as necessary to fulfill the purposes outlined in this policy or as required by law. Once the information is no longer required, it will be securely disposed of following PHIPA’s regulations, ensuring it cannot be accessed by unauthorized individuals.

Safeguarding Personal Health Information

We take the following steps to protect personal health information:

  • Implementing physical security measures, such as locked filing cabinets and restricted access to offices.
  • Using encryption and secure passwords to protect electronic data.
  • Training staff on privacy policies and ensuring they understand their responsibilities regarding PHI.

Consent

We will seek informed consent from clients before collecting, using, or disclosing their PHI, except where otherwise permitted by law. Consent may be implied (e.g., when bringing an animal in for care) or explicitly obtained where needed, such as sharing information with other veterinarians or insurers.

Complaints

If a client has concerns about our handling of their personal health information, they may contact us directly to resolve the issue. If the concern cannot be resolved, clients may contact the Office of the Information and Privacy Commissioner of Ontario (IPC) to file a complaint.
